Hive5 Drop-Off Program Waiver and Participation Agreement
1. By enrolling my child in a Hive5 drop-off program (after-school, camp, class, or workshop) at 43 First Commerce Drive, Unit J2, Aurora, ON, I agree to the following:
- I understand that Hive5 will provide supervision for my child only during the registered program hours, beginning at check-in and ending at scheduled pick-up time. Responsibility for my child remains with me before check-in and after pick-up.
- I acknowledge that program activities may include STEAM projects, robotics, crafts, indoor play, and physical activities, all of which carry inherent risks such as falls, minor injuries, and interactions with other participants.
- I voluntarily accept these risks and allow my child to participate.
- I agree that Hive5, its owners, employees, and volunteers shall not be held liable for any injury, loss, or damage to my child or property, except as required by law.
- I confirm that I have disclosed any medical conditions, allergies, behavioral concerns, or special needs that may affect my child’s participation.
- In case of emergency, I authorize Hive5 staff to seek medical attention for my child if necessary, and I accept responsibility for any related costs.
- I understand that:
• My child must be picked up on time
• Hive5 is not responsible outside program hours
• Only authorized individuals may pick up my child - Hive5 reserves the right to remove a child from the program if behavior is unsafe or disruptive, to ensure the safety of all participants.
- I give permission for Hive5 to take photos/videos of my child for educational or promotional purposes, unless I notify Hive5 in writing.
- By signing below, I confirm that I have read, understood, and agree to this Waiver.
- By entering Hive5 Innovative STEAM Park Playground or registering for any Hive5 program located at 43 First Commerce Drive, Unit J2, Aurora, ON, I acknowledge and agree to the following terms. Selecting “I Agree,” signing this form, or entering the facility constitutes a legally binding agreement.
2. Assumption of Risk
I acknowledge that participation in Hive5 activities—including but not limited to indoor playground use, STEAM activities, physical movement, outdoor play, swimming, gymnastics, bus transportation, and off‑site excursions—carries inherent risks such as:
Falls, collisions, or equipment-related injuries
Interactions with other children
Property damage
In rare cases, serious harm
I voluntarily assume all risks associated with my child(ren)’s participation and presence at Hive5.
3. Release of Liability
To the fullest extent permitted by law:
I release and hold harmless Hive5 Kids Inc., its owners, directors, employees, instructors, volunteers, and agents from any injury, loss, damage, or expense suffered by me or my child(ren), including damage to personal property, except where caused by gross negligence.
I understand that Hive5 staff may seek emergency medical treatment for my child if necessary, and all related costs remain my responsibility.
4. Behaviour & Dismissal Policy
I understand that Hive5 maintains a zero‑tolerance policy for unsafe or aggressive behaviour. My child may be immediately dismissed from camp or asked to leave the facility if they:
Engage in physical aggression
Bully or harass others
Damage property
Refuse to follow safety instructions
Pose a safety risk to themselves or others
I agree that:
No refund will be issued in the event of dismissal due to behaviour.
I am responsible for any costs resulting from damages caused by my child.
5. Refund Policy (For Registered Programs)
I acknowledge and agree to the following refund structure:
30+ days before the camp week: Full refund minus 10% administrative fee
8–29 days before the camp week: 60% refund
7 days or less before the camp week: No refund
After camp begins: No refunds for missed days, illness, early withdrawal, or dismissal
Refund eligibility is based on the date a written request is received at [email protected].
6. Program & Schedule Changes
Hive5 may modify schedules, activities, instructors, or excursions when necessary.
If Hive5 cancels a camp week, a full refund will be issued.
7. Media Release
Unless I provide written notice otherwise, I grant permission for Hive5 to photograph or record my child for promotional purposes.
8. Rules & Equipment Use
I confirm that my child(ren) are physically able to participate in playground and program activities.
I agree to ensure safe and appropriate use of all equipment.
I agree to follow all posted rules and instructions provided by Hive5 staff.
9. Binding Agreement
By signing below or selecting “I Agree,” I confirm that:
I have read, understood, and agree to all terms in this Combined Waiver and Parent/Guardian Agreement.
This agreement is legally binding and equivalent to a signed document.
