Hive5 Drop-Off Program Waiver and Participation Agreement

By enrolling my child in a Hive5 drop-off program (after-school, camp, class, or workshop) at 43 First Commerce Drive, Unit J2, Aurora, ON, I agree to the following:

• I understand that Hive5 will provide supervision for my child only during the registered program hours, beginning at check-in and ending at scheduled pick-up time. Responsibility for my child remains with me before check-in and after pick-up.
• I acknowledge that program activities may include STEAM projects, robotics, crafts, indoor play, and physical activities, all of which carry inherent risks such as falls, minor injuries, and interactions with other participants.
• I voluntarily accept these risks and allow my child to participate.
• I agree that Hive5, its owners, employees, and volunteers shall not be held liable for any injury, loss, or damage to my child or property, except as required by law.
• I confirm that I have disclosed any medical conditions, allergies, behavioral concerns, or special needs that may affect my child’s participation.
• In case of emergency, I authorize Hive5 staff to seek medical attention for my child if necessary, and I accept responsibility for any related costs.
• I understand that:
  • My child must be picked up on time
  • Hive5 is not responsible outside program hours
  • Only authorized individuals may pick up my child
• Hive5 reserves the right to remove a child from the program if behavior is unsafe or disruptive, to ensure the safety of all participants.
• I give permission for Hive5 to take photos/videos of my child for educational or promotional purposes, unless I notify Hive5 in writing.
• By signing below, I confirm that I have read, understood, and agree to this Waiver.
