Hive5 Walk-In Playground Waiver and Parent Supervision Agreement

By entering Hive5 Innovative STEAM Park Playground at 43 First Commerce Drive, Unit J2, Aurora, ON, I acknowledge and agree to the following:

• I understand that Hive5 is a recreational indoor playground facility and does not provide drop-off child care for walk-in visitors. I confirm that I will remain on-site at all times and that I am fully responsible for the supervision, safety, and behavior of my child(ren) during our visit.
• I understand that participation in playground activities involves inherent risks, including but not limited to falls, collisions, use of play equipment, and interactions with other children. These risks may result in injury, property damage, or in rare cases, serious harm.
• I voluntarily assume all risks associated with my child(ren)’s participation and presence at Hive5.
• I agree that Hive5, its owners, employees, and volunteers shall not be held liable for any injury, loss, or damage suffered by me or my child(ren), including damage to personal property, to the fullest extent permitted by law.
• I confirm that my child(ren) are physically able to participate and that I will ensure safe and appropriate use of all equipment.
• I agree to follow all posted rules and instructions provided by Hive5 staff.

By signing below, I confirm that I have read, understood, and agree to this Waiver.
