Permission to attend Gymnastics Excursions:
All costs are included in your camp registration. Please provide: clothes appropriate for gymnstics & water bottle. Two gymnastics coaches will be on duty and our Hive5 teachers will oversee as well. By checking the box below you acknowledge the information below and to: a) give consent and permission or b)not give consent and permission, for your child to attend. Campers who do not attend will remain at the Centre with staff and do an alternate activity.
CONSENT & ACKNOWLEDGEMENT OF RISK:
- As parent and/or legal guardian, I remain legally responsible for any personal actions taken by the minor participant I am registering for Hive5 STEAM Summer Camp.
- I agree on behalf of myself, my child named herein, or our heirs, successors and assigns, to hold harmless and defend Hive5 Kids Inc., its officers, directors and agents, and any other representatives associated with the event, from any and all actions, claims, demands, damages, costs, expenses and all consequential damage arising from or in connection with my child attending the event or in connection with any illness or injury or cost of medical treatment in connection therewith, and I agree to compensate Hive5 Kids Inc., its officers, directors and agents, or representatives associated with the event for reasonable attorney’s fees and expenses arising therewith.
- I accept transportation via the Hive5 Bus for this activity.
- I will be responsible for any costs caused by child’s failure to abide by established rules and respect.
- I acknowledge that it is my duty to advise Hive5 Kids Inc, of any medical/health concerns of my child that may affect participation.
