Hive5 Registration Agreement & Waiver

1. Parent/Guardian Responsibility & Supervision

• I understand that Hive5 is a recreational indoor playground and does not provide drop‑off child care for walk‑in visitors.

• I confirm that I will remain on-site at all times during walk‑in visits and am fully responsible for supervising my child(ren), including their safety, behaviour, and use of equipment.

• For registered programs (including Summer Camp), I confirm that all medical, behavioural, emergency contact, and authorized pickup information provided is accurate and complete.

• I understand that it is my responsibility to update Hive5 regarding any changes.

2. Assumption of Risk

I acknowledge that participation in Hive5 activities—including but not limited to indoor playground use, STEAM activities, physical movement, outdoor play, swimming, gymnastics, bus transportation, and off‑site excursions—carries inherent risks such as:

• Falls, collisions, or equipment-related injuries

• Interactions with other children

• Property damage

• In rare cases, serious harm

I voluntarily assume all risks associated with my child(ren)’s participation and presence at Hive5.

3. Release of Liability

To the fullest extent permitted by law:

• I release and hold harmless Hive5 Kids Inc., its owners, directors, employees, instructors, volunteers, and agents from any injury, loss, damage, or expense suffered by me or my child(ren), including damage to personal property, except where caused by gross negligence.

• I understand that Hive5 staff may seek emergency medical treatment for my child if necessary, and all related costs remain my responsibility.

4. Behaviour & Dismissal Policy

I understand that Hive5 maintains a zero‑tolerance policy for unsafe or aggressive behaviour. My child may be immediately dismissed from camp or asked to leave the facility if they:

• Engage in physical aggression

• Bully or harass others

• Damage property

• Refuse to follow safety instructions

• Pose a safety risk to themselves or others

I agree that:

• No refund will be issued in the event of dismissal due to behaviour.

• I am responsible for any costs resulting from damages caused by my child.

5. Refund Policy (For Registered Programs)

I acknowledge and agree to the following refund structure:

• 30+ days before the camp week: Full refund minus 10% administrative fee

• 8–29 days before the camp week: 60% refund

• 7 days or less before the camp week: No refund

• After camp begins: No refunds for missed days, illness, early withdrawal, or dismissal

Refund eligibility is based on the date a written request is received at [email protected].

6. Program & Schedule Changes

• Hive5 may modify schedules, activities, instructors, or excursions when necessary.

• If Hive5 cancels a camp week, a full refund will be issued.

7. Media Release

Unless I provide written notice otherwise, I grant permission for Hive5 to photograph or record my child for promotional purposes.

8. Rules & Equipment Use

• I confirm that my child(ren) are physically able to participate in playground and program activities.

• I agree to ensure safe and appropriate use of all equipment.

• I agree to follow all posted rules and instructions provided by Hive5 staff.

9. Binding Agreement

By signing below or selecting “I Agree,” I confirm that:

• I have read, understood, and agree to all terms in this Combined Waiver and Parent/Guardian Agreement.

• This agreement is legally binding and equivalent to a signed document.
